Job Summary:

We are seeking a Direct Care PRP Counselor to join our team and support youth and adults with mental illness in enhancing their quality of life. As a PRP Counselor, you will provide essential services to help clients achieve their rehabilitation goals with daily living skills, personal hygiene, organization, building supportive networks, and all while promoting a positive and supportive living environment. The ideal candidate is patient, compassionate, reliable, and committed to fostering independence and empowerment for individuals in need.

Responsibilities:

1. Meet with clients for individual and group sessions;
2. Participate in all treatment planning, communicate consistently with clinical staff, Program Director, and clients' families;
3. Attend and complete Ascend's required training;
4. Maintain contact with the client's family and treatment team;
5. Implementation of rehabilitation activities;
6. Assist clients in achieving their individual goals by teaching and supporting them while linking them to various community resources;
7. Maintain quality records and timely documentation.

Qualifications:

1. 2 years experience working with mentally ill children or adults;
2. Complete & pass a federal CJIS background check;
3. Valid driver's license and reliable transportation;
4. Ability to adhere to all health and safety guidelines;
5. Excellent communication and interpersonal skills;
6. Ability to maintain quality records and timely documentation;
7. Compassionate, respectful, ethical.
